Please call the Dean of Student’s Office at 203-582-8735 if you have questions about which form to complete. These forms are not designed as an emergency response notification process, and staff typically review and respond to reports during business hours.
Title IX
Students, faculty, or staff who believe they have experienced or witnessed an incident of gender-based discrimination and/or sexual misconduct related to the Title IX Policy Against Gender-Based Discrimination and Sexual Harassment may report incidents here.

Student of Concern Referral to CARE
Use this form to provide any information regarding a student’s health and well-being, situational concerns or concerning behaviors.

Use this form for the mandatory reporting of all potential violations of the Academic Integrity Policy by Quinnipiac students.
Bias Incident Reporting
Students, faculty, or staff who believe they have experienced or witnessed an incident of discrimination and bias-motivated behavior related to the Discrimination, Discriminatory Harassment, and Bias-Motivated Acts and Behavior Policy may report incidents here.

Student Conduct and Community Standards
Use this form to report potential violations of the Code of Conduct on or off campus. All Quinnipiac students are expected to know and abide by the policies, standards and expectations of Quinnipiac University. Any student reported to have violated this code will have their behavior addressed through the student conduct system. This form can be utilized to report incidents involving individual students and student organizations.

Thrive (for faculty)
Faculty should use this form to report any retention and/or student-success related concerns they believe should be shared with a student's major adviser, The Learning Commons, or other members of a student's success team. This includes information regarding absences, a decline in performance, poor grades, and/or other behavior that will impact a student's academic success or ability to remain at Quinnipiac.
